Job Summary
The
Forensic Re-Entry Services Team (FRS Team) provides an intensive intervention treatment
team for adult offenders who suffer from severe mental illness and co-occurring
substance abus e disorders that are
discharged from the jail to the community.
FRS Team clients receive a comprehensive effort to connect them to
appropriate health and community services for mental health treatment, physical
health treatment, food and clothing assistance, transportation assistance, drug
and alcohol recovery, hous ing and
vocational services. This approach is
designed to assist clients in their transition to the community to avoid
criminal recidivism, reduce harm from substance abus e,
and recover functioning while managing mental illness. The TMHA Personal Services Specialist works
as a team member with the Behavioral Health Department’s Community Treatment
Providers. The TMHA Personal Services
Specialist will be responsible for providing a “bridge” for individuals leaving
the jail in the form of assessment and referral to all appropriate health and
community services and supports in addition to “short-term case management”
during this transition.
Minimum Requirements
Bachelor’s
Degree in a related Human Services field and at least 6 months mental health
experience OR at least two years mental health experience. Ability to handle intense situations, display
good judgment and decision making skills, and remain calm and clearheaded under
pressure. Must have a strong commitment
to the right and ability of each person with a severe and persistent mental
illness to live in a safe living environment; develop a meaningful role in the
community; and have access to competent and continuous supports and
services. Must have a strong commitment in
clients’ rights to self-govern and create their own choices. Strong interpersonal skills and oral/written
communication skills; well defined organizational and time management
skills. Must use reasonable precautions
in the performance of one’s duties and adhere to all applicable safety rules
and practices while acting in such a manner as to assure at all times, maximum
safety to one’s self, fellow employees, clients, and other persons contacted
through TMHA business. Conditions of
employment include: must be at least 18 years of age; possess a valid CA
Driver’s License, minimum 2 years driving experience, reliable transportation,
proof of auto insurance, and DMV driving record that meets TMHA’s current
vehicle insurance requirements. May be
required to utilize personal vehicle and auto insurance. At the time of hire, other conditions of
employment are to comply with a T.B. skin test or chest x-ray indicating a
negative tuberculosis result, and receipt of a criminal record clearance
through fingerprinting. Must be able to
successfully complete First Aid and CPR training.
Compensation
Full-Time~ 40 hours. Entry level $15.36 per hour, wage negotiable based on education and/or experience. Position is eligible for benefits package.
